The COA team has transitioned all writeable PDF forms to online forms. These are accessible via the COA Intake Portal. All questions and requests for the COA team can be submitted through the portal.
MEDICAL SCHOOL - All YD, CC, PG, PJ, LA, SC, RC requests from Medical School departments will be routed to the YSM Controller’s team for review by the Intake Portal before they are submitted to the COA team. If your request is approved, the YSM Controller’s team will route your request to COA. If you wish to consult on COA design before submitting through the portal, please begin with ysmcontroller@yale.edu.
The forms in the COA Intake Portal are set up to handle single-segment requests. If you have a large volume of requests, please DO NOT complete a separate online form for each item. Instead, please complete one of the following excel templates. The template can be attached to your online portal request as follows:
- Type of Request = COA Segment Request Forms
- Which Segment – select your segment type
- COA Segment Request Type – select the option that references “Multiple”
- Template – Requesting Multiple Cost Centers, Edits or Reorganization
- Template – Requesting Multiple Projects or Edits
- Template – Multiple Assignees
For Segment Closures – please use the Closure Dashboard reports and complete the Required Checklist. The checklist must be uploaded as part of your Portal request.
