Changes to immunization documentation requirements

Yale Health wishes to bring to your attention some changes to the immunization documentation requirements that will be updated in the TMS system. These immunizations only impact employees working at the Schools of Medicine and Nursing, and Yale Health.
Currently, employees have an option to submit their immunization data (without documentation from a physician) using an online form linked to TMS. As of October 15, 2019, this online form will no longer be available. Employees will be required to submit actual documentation of their vaccine or titer history, along with their NetID, to fulfill their immunization requirements to Employee Health. They will need to email all documentation to Employee Health at employeehealth-business @yale.edu.
This new requirement is not retroactive and will not affect anyone who is currently compliant with their immunization requirements. Starting October 15, 2019, all new employees and those who are currently noncompliant, will be required to submit immunization documentation to Employee Health. This change is based on Centers for Disease Control and Prevention’s (CDC) guidelines that recognize the importance of protecting healthcare workers from vaccine preventable illnesses and advise that all health care workers should have immunization documentation on file.
Yale Health encourages any employee, who previously self-attested to their immunizations, to submit their actual documentation as well so that it can be recorded.
