IMPORTANT: All protoco​l-related paperwork must be submitted electronically as an Adobe Acrobat Reader/pdf document to iacuc@yale.edu.

Below are applications and forms used to obtain IACUC approval. A description of each form is noted below to assist you in determining which form will best fit your needs. Certain forms are prepackaged as “Application Packs” below which provide all necessary forms needed to complete the application process. All individual forms included in the Application Pack are noted in the Individual Form column. If you do not need to download an entire Application Pack and would only like to download an individual form you may choose any form from the Individual Forms column.

Individual Forms

Animal Adoption Agreement This agreement is required when adopting an animal that is no longer being used for research.
Annual Protocol Review  This form is required to review a protocol in its 2nd and 3rd year.

Application to House Animals in Non-YARC Managed Facilities

This form is required when requesting to house animals outside the YARC vivarium.

Lab Housing Daily and Weekly Log

This form is required to be completed when approval for housing animals outside the YARC vivarium has been approved by the IACUC.
Lab Housing Intent Form This form is required to be completed prior to housing animals outside the YARC vivarium as approved by the IACUC. 

Modifications to an Approved Protocol

Modifications Instructions   

This application is required when making changes to an already approved protocol. Changes include, but are not limited to: increase in numbers of animals, change in anesthetic, adding a new procedure, change in personnel. For additional requirements, see the IACUC Policy for Significant Changes to approved research.

Modification Form - Change in Analgesic Formulation

This application is specific to requesting a change in the analgesic formulation in currently active IACUC protocols.

Protocol for Permission to Have Custom Antibodies Manufactured

Approved Contract Labs

Custom Antibody Production Contract Laboratory Animal Use Agreement

This application is required for the purchase of custom-made anti-bodies from an APPROVED CONTRACT LAB.
Protocol for Use of Live Vertebrate Animals in Field Research This application is required for any live animal interaction in field research.
Qualification of Personnel This form must be completed for all personnel listed on a protocol, or those to be added to a protocol. Protocols or modifications will not be approved without submission of this form.
Request to Use Hazardous Agents If a research/teaching protocol involves the use of hazardous agents, or hazardous agents will be added to an already approved protocol this form must be submitted along with the application. Protocols or modifications will not be approved without submission of this form.
Special Instructions Please complete this form for circumstances that require special instructions for maintaining animal(s) health or to meet experimental needs. (Please note, this form is currently compatible with Internet Explorer 6).
