Livongo
People living with diabetes know the drill. They need to make smart food choices, stay at a healthy weight, lower their stress, adopt a more active lifestyle and manage the ups and down that come with this chronic condition. It’s a lot to do.
Livongo is a program which offers the tools and personalized support to make living with diabetes easier. The Livongo program provides a blood glucose meter, unlimited strips, and individualized coaching. The Livongo meter automatically uploads your readings to your private account, which can also be accessed through the Livongo mobile app on your phone.
The Livongo program is 100% voluntary and offered at no cost.
The program is offered at no cost and is available to you and your eligible dependents 13 and older who have diabetes and have coverage through Yale Health or Aetna health plans.
Program Benefits
- More than a standard meter: The Livongo meter is connected to your private account, provides real-time tips and automatically uploads your blood glucose readings to your private account, making log books a thing of the past.
- Unlimited strips at no cost to you: Get as many strips and lancets as you need with no hidden costs. When you are about to run out, Livongo ships more supplies, right to your door.
- Coaching anytime and anywhere: Livongo expert coaches are available anytime via phone, text, and the Livongo mobile app to give you guidance on your nutrition and lifestyle questions.
