Meet Your Health Coach
Adriana, Courtney, Chanel, Kim, Sonia and Trish are Registered Nurses with experience helping individuals make changes in their health behaviors and manage their health issues. Your Health Coach will help you develop an individualized program to achieve goals for healthy living regardless of whether you are ready to change or just thinking about it.
Your Health Coach can also assist you in understanding your medical diagnosis, your medical condition, and your doctor’s treatment plans so you can make important changes to achieve optimal health and well-being. Even if you don’t have a medical condition, your Health Coach can help you make important changes in your health behaviors (i.e., exercise and nutrition) to help you improve your health and prevent the development of certain medical conditions.
Your Health Coach is available to meet with you conveniently on campus or by telephone.
Health coaching is a benefits program offered by Yale University in partnership with TrestleTree, an accredited health transformation organization. The program provides you with free, confidential assistance to achieve your health goals.
What is a Health Coach?
Have you ever wanted to improve your health but didn’t know where to start? A Health Coach is a healthcare professional who partners with you to transform your health goals into action. Your Health Coach will provide guidance, support, resources, and help you to overcome obstacles that may be keeping you from realizing optimal health.
The health coaching program provides you the opportunity to work one-on-one with a Health Coach to improve your health. You can work with a Health Coach in the following areas:
- Diabetes Management
- Weight Management
- Tobacco Cessation
- Stress Management
- Management of other Health Conditions

How does health coaching work?
- You can talk with your Health Coach in person or over the telephone through conveniently scheduled appointments.
- Appointments can range from 15 minutes to an hour. How long and how often you meet with your Health Coach depends on your individual needs.
- Your Health Coach will help you create a plan that is right for you to meet your health goals. Everything in the program is tailored to you.
- You receive written materials from your Health Coach that can help you decide where you want to go with your health and how to reach your health goals.
All health coaching is completely confidential and meets the strict requirements of federal and state laws and regulations. Your information will only be shared as permitted by law.
