If you do not elect to receive your Form 1095-C electronically, a copy of the 1095-C form will be mailed to your home address at the end of the month. Beginning February 2026, a printable version will be available through Workday by following the instructions below:
- Visit Workday and log in.
- Select Menu on the upper left corner, then select Benefits and Pay > Benefits > ACA Forms.
- Select the View/Print button to the right of your form. Please be patient. It may take a minute to process the request.
- Your form is now open as a PDF and you have the option to print the PDF or download the PDF to your computer.
