If you do not elect to receive your Form 1095-C electronically, a copy of the 1095-C form will be mailed to your home address at the end of the month. Beginning February 2024, a printable version will be available through Workday by following the instructions below:
- Go to the Workday website and log in.
- In the search box at the top left, type “My ACA Forms” and click on it when it appears below the search box.
- Click the View/Print button to the right of your form. Please be patient. It may take a minute to process the request.
- Your form is now open as a PDF. You have the option to print the PDF or download the PDF to your computer.
