Steps for Process After Being Approved for Reimbursement for Unpaid Post-Doctoral Fellows

You will receive an email from Ben Walter, from WorkLife and Child Care, who will be processing the reimbursement. Before Yale can process your childcare subsidy reimbursement, we need to set you up as a vendor. You will need to submit a tax form, confirm your payment method, and submit your phone number. You will be submitting all required information and documents to Ben directly. Depending on the payment method you select, you may need to submit additional forms. Check which documents listed below apply to you and send them to Ben Walter (email info provided below). This information is stored on a secure server and will not be shared with others.

Tax Form (one of the following):

  • If you are a US citizen or permanent resident: W-9
  • If you are not a US citizen or permanent resident: W-8BEN. Note: After you submit the W-BEN you will receive an email from the International Tax Office to complete the Foreign National Information System application (FNIS). You must complete the FNIS to receive the subsidy.

Payment Method (one of the following):

  • If you have a US address and would like a paper check mailed to you: no action is needed; Yale will mail you a check to the address listed on your tax form.
  • If you have a US bank account: ACH-EFT form, plus include one of the documents listed in Section 4 of the form.
  • If you have a non-US bank account: Wire Payment Setup Form, plus one of the listed documents in Section 4 of the form.

  • Before submitting, double-check that the forms are fully complete, and the information is correct. Missing or incorrect information could lead to delays in processing your childcare subsidy.

If submitting a Wire Payment Setup Form or ACH-EFT Form, don’t forget to sign at the bottom and submit one of the required pieces of documentation listed in Section 4. This documentation is required.
